Kentucky  Oversize permits are available in Kentucky for ten calendar days and must be secured before visiting the state. The good news is that, with a few exclusions, Kentucky now permits continuous running, 24/7, including weekends:

Restricted Travel: Accepted loads are not allowed to travel in Boone, Kenton, or Campbell Counties (greater Cincinnati area), Jefferson County (greater Louisville), Fayette County (greater Lexington), or at the Owensboro, KY 2155 Bridge (between KY and IN) Sunday through Friday. 
Restraints may apply for super loads, defined as loads more than 300,000 lbs gross.

Legal Dimensions and Routine Permission Limits

Kentucky has certain regulatory restrictions on trucks and specified legal dimensions.

Legal Dimensions: 53' trailer or load on Interstate and authorized routes, 65' truck and trailer on other highways; 5' rear overhang on 48' trailer
On Interstate and approved highways, 8'6"; on non-designated roads, 8';

Weight: 80,000 Gross (Single - 20,000, Tandem - 34,000, Tridem - 48,000). 

Length: 110' on 4-lane, 75' on 2-lane,

Weight : 5 axles - 96,000 lbs; 6 axles - 120,000 lbs (six axles and 7 axles on a two-lane);
On 4-lane with a booster axle instead of a tridem to go above around 134,000, 7 axles - 134,000 lbs*

Note: Loads over 15'6" height cannot pass through Kentucky. Should the carrier be able to identify safe pathways for such a cargo, delivery into Kentucky may be permitted once-only. Generally however, they won't let it.

Escort Needs

The weight and size of the transportation determine Kentucky's particular escort needs:

Length: over 120' - 3 escorts; overhang: above 10' - 1 escort (some routes may require two). 

On 4 or more lane split highways

Width: More than 12' one escort, More than 14' could need for two escorts. More than 16 - three escorts
One front escort with height pole above 14'11".

On 2-lane roads,

Length: Between 75' and 85' one escort, Over 85' one front and 2 rear escorts; 

Overhang: Over 10' - 1 escort (certain roads may need 2 escorts) 
 Width: More than 10'6" to 12' one escort. 2 escorts covering

Height over 12' to 14' Over 14'11": 1 front escort with height pole 

Miscellaneous Needs

Although they may be transported with the load, blades, buckets, and other equipment exceeding 14' width have to be removed. Even in cases with an overweight load, smaller items may be separated and pulled alongside.
Every cargo more than 10'6" calls for "Oversize cargo" signs.
Every four corners of any broad load and extremities must be marked with flags. Over-length loads have to be reported; there is no indication needed.
Should the front overhang be over 10 feet, the towing vehicle's power unit has to have an amber strobe or flashing light installed.
